" SSS Maternity Benefit Claim Checklist (Postpartum Filing)

" 1. Maternity Notification (MAT-1)

• This must have been submitted before childbirth
• If you didn’t submit it, SSS may deny or delay your claim

" Can be submitted online via My.SSS portal or SSS Mobile App

" 2. Maternity Reimbursement Application (MAT-2 Form)

• Download and fill from: https://www.sss.gov.ph
• Submit to SSS branch or online if applicable
  

" 3. Valid ID (any of the following)

Submit 1 original and 1 photocopy of any of these:

• UMID (Unified Multi-Purpose ID)
• DriverTMs License
• Passport
• Postal ID, VoterTMs ID, etc.
  

" 4. Proof of Childbirth or Miscarriage

Depending on your case, include:

• Live birth:
  • PSA/NSO or Certified True Copy of Birth Certificate
  • Clinical or hospital record (if home birth, must be signed by midwife/attending health worker)
• Miscarriage or abortion:

" 5. Proof of SSS Contributions

Not always required if your records are updated, but prepare:

• Latest SSS contribution printout (from your online account or from a branch)
  

" 6. Employer Certification (if employed)

Only if you’re employed during pregnancy:

• Statement of no advance payment (if they didnTMt advance the benefit)
• L-501 Form (Specimen Signature Form)
  

" 7. Bank Account Details or Disbursement Account Enrollment

SSS no longer issues checks. You must:

• Enroll your bank or e-wallet in the Disbursement Account Enrollmentⓘ Module (DAEM) via My.SSS
  

Optional:

• Marriage Certificate (if claiming under husbandTMs name or with surname discrepancies)
