What the PhilHealth maternity benefit does

Why prepare CSF and CF1?

The Claim Signature Form (CSF) and Claim Form 1 (CF1) contain member, patient, employer, and claim details used in the facility's PhilHealth process. Preparing them in advance can help reduce encoding mistakes, but the hospital or clinic may still review the forms and require signatures or additional information.
