Important — receipt must be CTC: Use a Certified True Copy (CTC) of the corresponding Official Receipt (OR) or Acknowledgement Receipt (AR). An ordinary photocopy may lead to rejection or a request to resubmit.
Solo Parent document
Add one of the following LGU-issued documents to the continuous PDF:
- Valid Solo Parent ID; or
- Certification/e-Certification of eligibility if the ID is not yet available.
The document must be signed by the Social Worker and City/Municipal Mayor. The delivery date must fall within its validity period. For a first-time Solo Parent, the document must have been issued within 6 months from the delivery date.
No extra SSS form for a regular miscarriage claim
For an ordinary online miscarriage / ETP claim, the important part is submitting the required medical evidence. You do not need the separate MBA/MBRA “Special Cases” form just because the maternity event was a miscarriage.
Doctor details: Medical documents should show the physician’s name and PRC license number, and the applicable pregnancy/termination records must be duly signed by the physician or municipal health officer as required.
Electronically issued medical document? Include the Official Receipt of the procedure in the continuous PDF.
What should you ask HR for?
SSS requires the Certificate of Separation from Employment to state:
- your effective date of separation; and
- that no advance payment of maternity benefit was granted by the employer.

Need the Affidavit of Undertaking?
Use the official SSS Affidavit of Undertaking — Non-Receipt of Advance Payment for Sickness/Maternity Benefit. The form covers qualifying reasons such as AWOL, strained relations, company closure, unavailable former-employer records, and other SSS-listed situations.
The Affidavit of Undertaking must be administered by the applicable authorized SSS branch official/employee or foreign representative before submission.

“MAT-2” is the term many SSS members still use for the maternity benefit claim stage after childbirth, miscarriage, emergency termination of pregnancy, stillbirth, or another covered maternity event.
In the current online process, individual members file a Maternity Benefit Application (MBA), while employers use a Maternity Benefit Reimbursement Application (MBRA) when applicable.
This page uses “MAT-2” in the title because it remains a familiar search term, while the guide also explains the current filing terminology.
SSS MAT-2 requirements
Exact supporting documents depend on the maternity event and your member situation. Use the guided tool above to identify the document set that applies to your case and arrange it in the correct continuous-PDF order.
For the common local live-childbirth route, prepare the applicable birth record together with the corresponding receipt. This page uses the following rejection-safe continuous-PDF sets:
- Within 6 months: LCR-registered Certificate of Live Birth + CTC of the LCR OR/AR.
- Separated within 6 months before birth: the 2 documents above + Certificate of Separation stating the effective separation date and no advance maternity payment.
- AWOL / cannot obtain employer certificate: the 2 documents above + applicable Affidavit of Undertaking.
- More than 6 months after birth: PSA-issued Certificate of Live Birth + CTC of the corresponding OR/AR.
Qualified Solo Parent claiming 120 days: add either a valid Solo Parent ID or Certification/e-Certification of eligibility issued by the LGU.
When the SSS upload field asks for continuous scanning, put the applicable documents into one PDF in the same order shown by the tool above.
- Within 6 months from delivery: LCR-registered Certificate of Fetal Death (COFD) + CTC of the LCR OR/AR.
- More than 6 months from delivery: PSA-issued Certificate of Fetal Death (COFD) + CTC of the corresponding OR/AR.
- Recently separated: add the applicable Certificate of Separation stating the effective separation date and no advance maternity payment.
- Cannot secure the separation certificate: add the applicable SSS Affidavit of Undertaking instead.

Phone photo is okay. If your document is a clear JPG, PNG, WebP, or another image your browser can open, the tool will turn it into a PDF page automatically. No separate image-to-PDF website is needed.
