Termination Certificate

A Termination Certificate is an official document issued by the employer stating the employee's reason for job separation. It is a mandatory requirement for filing an unemployment benefit claim. The document must indicate that the separation was involuntary and must include the date of separation, cause, and employer signature.

This certificate verifies eligibility and prevents fraudulent claims. Without it, SSS will not process unemployment benefit applications.
